Requirements
Your jobs will not be completed without the following:
For Test Grading (red or teal forms)
  1. All forms must have the ID number section completed with the same number of digits but unique IDs for each student. (1 digit all the way up to 9 digits are allowable. Each set to be run must have the same number of digits in the ID section for all forms to be graded as one session.)
  2. All forms, including keys, must have the Test Form section completed. (This indicates which version of the test was taken. Even if only one version was available, this section must still be completed.)
  3. Multiple answers on the key must be specified as "and" or "or" answers and noted in the special instructions below. ("And" means that all correct responses on the key must be present for the answer to be considered correct. "Or" indicates that only one of all answers indicated on the key is needed. If more than one answer is marked on an "or" type question, the machine will see this as a multiple mark response and it will not be graded.)
For Course Evaluations (pink forms)
  1. If you are planning to send pink evaluation forms to be scanned, please be sure to include your data disk with the appropriate data saved to the disk.
  2. Special coding is necessary for the proper processing of these forms. If you have never dealt with this particular form before, please contact Stephanie Stephenson at the CELT Office at (260) 481-6354 or scantron@ipfw.edu for assistance before passing the forms out to your instructors.
