Application Instructions

As of May 2, 2011 license applications will only be accepted electronically using the State’s new Licensing Verification & Information System (LVIS).  Also, educators will be able to download and print their new licenses.  Licenses will no longer be mailed.

Application InstructionsLicensing 

Go to www.doe.in.gov/educatorlicensing and click on the "LVIS" information link.  From there, you may select from LVIS manuals and training webinars to learn how to create your profile and apply for your license.

Note: Transcripts, CPR card* and PRAXIS scores (if applying for an original license), and employment verification letter(s) (if applicable) must be submitted to the IPFW licensing advisor prior to submitting your online application request.  These documents may be submitted to the following address:

*Please note that there have been recent changes to the CPR requirement for Indiana educators.  The new requirement includes the need for automated external defibrillator (AED) training for initial licensing.  Beginning July 1, 2012, this requirement must be met for all renewals as well.
